Students

Please follow all the rules and regulations for observing patient care at the University of Florida as laid out on the UF Privacy Website.

It is the responsibility of the requesting shadower to complete the Shadowing Request Form below and upload files no later than three weeks before the requested start date. International students for foreign medical graduates must complete all forms no later than six months before the requested start date.

Please Note: We do not accept shadowing from non-medical students between the months of July-December. If you request to shadow during that time frame, your request will be denied.

You will need to upload the following documents along with your Shadowing opportunity form:

  • HIPAA General Awareness (PRV800) Training Certificate (screen shot acceptable)
  • Driver’s license
  • Emergency Contact (Name / Phone Number)

If you do not submit this information, we will not be able to process the request.
